Her knee cost $90,000 at home. In Penang, the whole trip was $17,000
Wasilla, Alaska.
Mary Percak-Dennett owned a tanning salon there, and by 2006 her health insurance cost $1,421 a month.
So she dropped it. (The policy excluded her bad knee anyway, which anyone who has read the fine print on a preexisting condition will understand.)
Sound familiar? Nearly 3 million people left the ACA marketplace in the year ending early 2026, after the extra subsidies expired.
Mary’s knee had hurt for years. Her orthopedic surgeon warned she’d land in a wheelchair without a replacement, and the operation carried a $90,000 price tag.
Ninety thousand dollars… for one knee.
She was 50, self-employed and uninsured. So she started looking overseas, and she landed on Gleneagles Medical Centre in Penang, Malaysia.
In June 2007 she flew over. The new knee, round-trip airfare and a stay in a nice hotel came to $17,000, total.
In other words, the whole trip cost about one year of the premiums she’d stopped paying.
She came home without her cane and without the pain.
The staff all spoke English. Nurses fussed over her, and the surgeon patiently worked through her long list of questions.
“It’s the way care used to be,” she told Kiplinger’s Retirement Report.
That was 2007. What does the trip cost now?
Interesting… you can do it for less.
Sunway Medical Centre Penang, a newer hospital on the mainland side, lists a single total knee replacement package at RM26,000, about $6,400 at current exchange rates.
That covers the surgeon, the anesthesiologist, the implant, medicines and in-hospital physical therapy.
Five days and four nights in the hospital are included too.
Add a round-trip flight from the West Coast (about $1,000 on recent KAYAK fares) and ten recovery nights at the Sunway Hotel in Seberang Jaya, roughly $90 a night with breakfast.
Call it $8,300, all in.
(Once you’re walking again, George Town’s UNESCO-listed old streets are just across the Penang Bridge.)
Back home, a hospital knee replacement without insurance runs $35,000 to $60,000, according to CoveredUSA. A surgery center will do it for $20,000 to $30,000.

|
Spread over three years, $8,300 works out to about $230 a month before interest. And the same math on $35,000 is $970 a month.
But there are caveats.
Sunway carries Malaysia’s national accreditation (MSQH), not JCI. Gleneagles Penang, Mary’s hospital, holds the international seal, so get a written price from both.
The package price is good through December 31 for self-pay patients. It excludes complications, and the full deposit is due at admission.
Your surgeon decides when you fly home, not your ticket. And see your own doctor before you leave and again when you’re back… Mary’s Penang surgeon insisted on it.
Will it work for everyone? I have no idea.
It worked for Mary.

Luxdentica, Krakow
Implant-supported overdenture · September 2025 · bookimed.com
★ 4.8 Google, clinic average
In 2023, Henry lost every tooth in his upper jaw. He spent the next two years on a regular denture, and he says it dragged down his whole quality of life.
With a push from his wife, he looked abroad and chose Luxdentica in Krakow, Poland, for an implant-supported overdenture (a denture anchored to implants in the jaw).
The work took two trips. Dr. Dariusz Wilisowski placed the implants in December 2024 with an anesthesiologist on hand, and in May and June 2025 Dr. Radzikowska built the new teeth, making sure they wouldn’t bring back the underbite he’d had all his life.
Both doctors took extra time to walk Henry and his wife through the plan, and a patient coordinator, Klaudia, kept the appointments lined up.
Months later he chews with confidence and says the new teeth “feel better than my natural teeth.” He plans to keep going back for checkups.
His review lists no price. For reference, the clinic’s Bookimed page (prices updated March 2025) shows single implants at 1,500–3,034 and All-on-4 full arches at 5,800–9,200. Bookimed lists one accreditation for the clinic, from the International Congress of Oral Implantologists.

Hospitals say patients are putting off surgery
Four of the biggest for-profit hospital chains in the US (HCA, Tenet, Community Health Systems and Universal Health Services) told investors last week that scheduled surgery is slowing. HCA said same-facility outpatient surgeries fell 3.7% in the third quarter, while its marketplace-insured admissions dropped about 19%, with those patients turning up as uninsured instead. Executives blamed lost coverage after the extra ACA subsidies expired, plus rising deductibles and copays. Plenty of knees are on hold. If yours is one of them, ask your surgeon what a three-, six- or 12-month delay could mean before you decide. (Via Medical Daily, citing Becker’s Hospital Review.)

Manila opens an airport lounge for patients
The Philippines is making a play for medical travelers. As of September 2026, the Department of Tourism’s accredited network included 13 hospitals, 21 ambulatory clinics and 10 dental clinics, and six Philippine hospitals hold JCI accreditation. A new Medical Concierge and Lounge at NAIA Terminal 3 in Manila meets arriving patients and connects them with accredited providers. Dental work, eye care, rehab and fertility care are on the list of services officials want to grow. English is widely spoken, which matters when you’re signing consent forms. Check any hospital’s accreditation yourself before booking. This is one to watch. (Travel And Tour World)

The $30,000 fertility experiment
A STAT investigation published Nov. 2 looked at women who paid an Illinois broker, International Fertility Group of North America, for “three-parent” IVF abroad, mostly at a clinic in Tirana, Albania. STAT reported the package price rose from $20,000 to $30,000 this year, with medications and donor eggs often extra. The technique was developed to prevent rare inherited mitochondrial diseases. Scientists told STAT there is little evidence it helps older women conceive, and the main commercial programs have published no peer-reviewed results. The broker told STAT it doesn’t describe the treatment as guaranteed. Before you pay any clinic abroad, ask for the live birth rate per started cycle for women your age. (STAT)

Get the US price in writing too
Open enrollment for 2027 marketplace plans began Nov. 1 in most states, and premiums are rising again. If you end up paying cash for surgery, use a rule most people never hear about. Under the No Surprises Act, uninsured and self-pay patients are entitled to a good faith estimate when they schedule care or ask for one. If the final bill lands at least $400 above that estimate, you can dispute it. Nonprofit hospitals must also keep written financial assistance policies, so ask about those as well. Then compare the real US number with the one from abroad. Sometimes the gap shrinks… sometimes it doesn’t. (CMS)
